Cambridge is fortunate to have a collegial group of family physicians who practice in a variety of models:

Cambridge offers a collegial atmosphere for family physicians. This yields the advantage of intra-clinic CMEs, the choice of remaining "MRP" or admitting a patient to the hospital under hospitalist care, and an innovative on-call system where the family physicians are on-call once every 24 - 27 days and the choice of either "surgical assist - to assist with emergency surgeries" or "city-wide call - to back up the hospitalists."

Grandview Medical Centre Family Health Team:
Grandview Medical Centre Family Health Team is a Physician led multi-disciplinary Health Team servicing 28,000+ patients in the Cambridge and surrounding area. The Centre has used an integrated Electronic Medical Records system (EMR) since 1997 which allows for rapid sharing of pertinent health information between all health providers within the Family Health Team. The mandate for our Family Health Team continues to be:
"To provide our patients with high quality comprehensive medical care, in an accessible and timely manner that is patient focused, patient centered and delivered by a team of dedicated medical professionals"
Currently, we have 17 Family Physicians that practice at Grandview Medical Centre Family Health Team with a full complement of Nurses, Dietians, Pharmacists, Health Promoters, Administrative Staff and a comprehensive Mental Health Team that includes Social Workers and a Clinical Psychologist. Prior to becoming a Wave 1 Family Health Team in 2005, Grandview Medical Centre had been a Health Service Organization (HSO) since 1989 and became a Family Health Organization (FHO) in November 2006 when the HSO's dissolved.
Langs Farm Community Health Centre:
Langs Farm Village Association (LFVA) in Cambridge, Ontario is a non-profit, neighbourhood based organization with over 30 years of experience providing service to the community. The vision statement of the organization is "healthy and engaged communities where residents are valued and supported." Our mission statement is "to enhance community participation by providing programs and services for all ages."
The Langs Farm Community Health Centre (CHC) is located at 887 Langs Drive, Unit 1. Our hours of operation are Monday to Thursday from 8:30 to 8:00 PM and Fridays from 8:30 to 4:30 PM. CHCs are accredited, non-profit organizations governed by a volunteer Board of Directors that employ interdisciplinary teams of health providers: physicians, nurse practitioners, nurses, social workers, dietitians, health promoters, and others who are employed and paid by salary rather than through fee-for-service. Community Health Centres provide both primary health care and a range of health promotion, illness prevention and community development programs and services. CHCs have provided primary health care in Ontario for more than 30 years and are currently funded by Local Health Integration Networks.
The Langs Farm CHC provides primary care services for the Preston community and priority populations including a weekly clinic at the Bridges Shelter. Our physicians, nurse practitioners and nurses provide regular family checkups; physical exams; treatment of illness; prenatal examinations; health education and promotion and additional support services. Our social workers, dietitians, health promoters and community workers help promote good mental, social and physical health within the community by offering workshops on important social and health topics, and general wellness information on a one-to-one basis. In addition to working with volunteers and community partners, Langs also offers a wide variety of social and recreational programs at their Family Resource Centre, Resource Centre; Youth and Teen Community Centre.
Two Rivers Family Health Team:
The Two Rivers FHT is comprised of 3 clinic sites and 3 solo physician offices in Cambridge. Currently, we have 23 family physicians operating as Family Health Organization (FHO), and as a FHT which includes allied health staff located at the following sites - Hespeler Clinic (8 physicians), Cambridge Healthlink (7 physicians), and the Forbes Park Medical Centre (5 physicians.) We are a mix of about 2/3 female and 1/3 male physicians with a wide range of ages. On call time is flexible to accommodate medical commitments outside the office. The medical interests of the current physicians include hospitalist work, as well as Clinical Consultants to Chronic Care, Public Health, Long-term Care facilities, HIV/STD clinics, obstetrics and CMEs. Our Family Health Team includes interdisciplinary providers which currently includes, but will not be limited to nurse practitioners, nurses, clinical pharmacists, social workers, registered dietitians, specialist consultants, and administrative support staff.
We offer comprehensive primary care to the people of Cambridge, Ontario and the surrounding area. The current rostered population of Two Rivers FHT is over 30 000 patients and climbing - due to almost doubling in the number of physicians since becoming a FHT. Our patient geographic profile is mainly drawn from the city of Cambridge and North Dumfries; however, we also have many patients from Guelph, Kitchener, Waterloo, St. George, Ayr, Paris, and Aberfoyle.
Two Rivers is committed to providing patient-centred care with a focus on wellness, prevention, and effective management of acute and chronic disease/conditions. The Team is supportive of innovations and education in health care by creating a teaching and learning environment and has recently been selected as a clinical education site for the new medical campus in Kitchener.

Two Rivers emphasizes the provision of primary health care within its core business. A significant focus of this collaborative team is providing patient education in a variety of formats, including one-on-one consultations and group educational/teaching sessions aimed at illness prevention, wellness, and management of chronic conditions.
The staff at the offices are congenial and efforts are made throughout the year to pull our multi-site FHT together, with an annual activities - a summer barbecue, various fundraising events and Christmas festivities.
The use of information technology is an integral part of the communication system within the Team, and is vital to the successful integration of all members - the clinic sites are modern with an electronic medical records system since 2004 (NetMedical), computerized billing and patient scheduling.
Grand River Sports Medicine Centre:
Grand River Sports Medicine Centre (GRSM) is a multidisciplinary full-service sports medicine centre with facilities in Cambridge and Kitchener, Ontario, Canada. GRSM has a special interest in treating sports and exercise-related injuries through a comprehensive biomechanical assessment and a sport specific treatment plan by a registered or certified professional. Services include sports medicine physicians, physiotherapy, massage therapy, athletic therapy, pedorthics (custom foot orthotics), sports nutrition, chiropractic, osteopathy and sports bracing (custom & off the shelf).
GRSM's mission is to help athletes and active individuals achieve their sports and exercise goals. GRSM is committed to providing the best sports medicine services possible, with a compassionate and caring attitude. By focusing on the identification, treatment and prevention of exercise-related injuries and conditions, their multi-disciplinary team of knowledgeable and certified professionals enables clients to succeed. GRSM's ultimate priority is to help clients fulfill their active lifestyle pursuits.
North Dumfries Community Health Centre Satellite:
The North Dumfries CHC Satellite provides a range of primary health care services including health promotion, disease and illness prevention, and health promotion activities with an interdisciplinary team of health care providers, allied health staff and community health workers. Langs Farm, the sponsoring organization began providing outreach services to the North Dumfries community in 2004 and opened the satellite location in 2009. The North Dumfries CHC is located at 53 Hilltop Drive in Ayr, Ontario in North Dumfries Township. Our hours of operation are Monday and Thursday from 8:30 to 8:00 PM and Tuesdays and Wednesdays from 8:30 to 4:30 PM. The satellite is closed on Fridays. The catchment area for the CHC satellite includes the portion of North Dumfries Township found west of the Grand River as well as two communities in Oxford County and one community in Wilmot Township. The catchment has been defined based on information gathered from community consultation and health survey data and "natural" community use patterns for health care and social services. The total population of the catchment community is 12,000 and includes the communities of Ayr, Roseville, Drumbo, Canning, New Dundee, Plattsville, and Washington. Clyde and Branchton residents may access services at the Langs Farm CHC.

Cambridge Memorial Hospital is a thriving and vibrant 150 bed community hospital that proudly provides a full range of patient-focused, acute care services to more than 135,000 residents of Cambridge, North Dumfries Township, Deer Ridge and Doon. CMH is located in one of the fastest growing and strongest economic regions in southern Ontario, within easy access of Toronto, Hamilton and London. Cambridge provides a high standard of living with attractive residential areas, an excellent selection of schools, easy access to five major universities and three colleges, along with many recreational and cultural activities.
CMH is an affiliated teaching site of McMaster University School of Medicine and has welcomed more than 100 learners in placements in 2008 and has the distinction of being one of twelve healthcare organizations in Canada, to receive the RNAO Best Practice Spotlight Organization. In 2007, Cambridge Memorial Hospital installed an enterprise-wide PACS (Picture Archiving Communication System) along with a new 64 slice CT scanner. This system allows our medical staff to obtain information through a secure user access portal known as ROAM (Role Oriented Access Management). It is also the gateway for all remote users including, travelling and on-call physicians and staff, regional pathology labs, local clinics and medical specialists in other cities. PACS is just one of many innovative systems CMH has implemented over the last several years, others include CMH-PIP - our emergency department process improvement plan, regional transcription and dictation system, solar air heating and cooling project and a Pharmatrust drug dispensing kiosk in our ED.

Department of Family Medicine:
Membership in the medical staff of the Cambridge Hospital (CMH) represents a valuation and commitment to the principles and practice of community based health care. As health care delivery moves increasingly into the community for which it operates, where our patients actually live and work, it is increasingly important to have the experts in community based care at the relevant decision making tables and service provision sites. Family physicians are the major provider group that crosses those interfaces between home and hospital. As a true community hospital, CMH welcomes the presence and input of community providers at the table and on the wards.
The Department of Family Medicine at CMH is the organizational platform for family physicians to collectively organize, advocate, and credential their skills and activities both within and outside the Hospital. We are the discipline with the largest membership on medical staff, and we function as an integral part of the larger medical body.
CMH welcomes family physicians to act as MRP for their own inpatients. This is done in a variety of solo and group models. The Department coordinates rosters for collective overnight inpatient call, usually taken from home, and for after hours surgical assists. CMH offers further opportunities for active in-house care within the Emergency Department and the Hospitalist Progam.
The Department of Family Medicine conducts regular monthly rounds, on the second Tuesday of every month. The educational foci for the year 2009-2010 are selected areas of clinical excellence, and an examination of the changing face of the discipline.
Cambridge Academy of Medicine:
The Cambridge Academy of Medicine supports and encourages social interaction
amongst its members. We also promote continuing medical education by organizing and sponsoring lectures that are relevant to the practice of medicine in our area. In the past we have had specialists from London and Guelph who have spoken on topics which are rarely covered in other CME activities. These evenings also allow us to meet specialists whom we often only know by name.
Other events in the past have included a yearly clinical day, a yearly Christmas dinner with presentation of service awards, a golf tournament, and a bowling/pizza afternoon for doctors and their families. Each of these activities makes Cambridge a more enjoyable place to practice medicine and as such we support "Doctors 4 Cambridge" in its efforts in recruiting new physicians.
